Hi,
1. sometimes a paragraph text is missing after rendering (see attachment). This issue occurs mainly on the first line of a new page.
Any idea how to avoid this?

2. the TOC seems not to respect the correct hierarchy. All my headlines are H2. In the screenshot you'll see that they are not all in the same hierarchy. Any ideas on that?
